This Privacy Policy explains how we collect, use, disclose, retain, and protect personal information when you visit truesourcewellness.com, create an account, place an order, complete a required consent form, communicate with us, or otherwise use our website and services (collectively, the "Services").

- Contact and account information, such as your name, email address, telephone number, billing and shipping addresses, username, and password.
- Order and transaction information, such as products ordered, order history, payment status, shipping details, returns, refunds, and communications about an order. Payment card details are generally processed by our payment processor rather than stored directly by us.
- Consent-form information, including assumption-of-risk acknowledgments, checkbox initials, electronic signatures, dates, timestamps, and the version of the form you accepted. The current consent form does not request medical history, diagnoses, medications, treatment information, or other medical information.
- Communications and support information, including messages, requests, reviews, and other information you choose to provide.

When you use the Services, our website and service providers may automatically collect device and usage information such as Internet Protocol address, browser type, device type, operating system, pages viewed, referring URLs, approximate location derived from an IP address, access times, log data, and cookie or similar-technology identifiers.

If you provide a mobile telephone number and agree to receive text messages, we may send non-marketing messages related to your transaction or account, such as order confirmations, payment or consent-form notices, shipping updates, security alerts, and customer-service replies. We do not use this SMS program for marketing. Message frequency varies. Message and data rates may apply. Consent to receive texts is not a condition of purchase where prohibited by law. You may reply STOP to opt out and HELP for help, or contact support@truesourcewellness.com. Opting out of texts does not prevent us from contacting you through another permitted channel about an active order, account, safety issue, or legal obligation. Wireless carriers are not liable for delayed or undelivered messages.
5. Cookies and Similar Technologies
We and our operational service providers may use cookies and similar technologies that are necessary to operate the website, keep you signed in, maintain a shopping cart, prevent fraud, remember preferences, and understand basic site performance. Browser settings may allow you to block or delete cookies, but essential features may not work correctly if you do. We do not use personal information for targeted advertising.
6. Consent Forms and Record Retention
A completed consent form is required for each order. We keep the completed form, electronic signature, initials, timestamps, related transaction information, and the version accepted as part of our business and legal records. If a consent form is updated, you may be required to complete and sign the new version before placing another order.
We retain personal information only for as long as reasonably necessary for the purposes described in this Privacy Policy, including providing the Services, meeting tax and accounting duties, maintaining transaction and consent records, resolving disputes, enforcing agreements, preventing fraud, and complying with applicable law. Retention periods vary by record type and legal requirement.
A deletion request does not require us to delete information that we must or are permitted to retain for legal compliance, recordkeeping, fraud prevention, security, dispute resolution, or the establishment, exercise, or defense of legal claims. For those reasons, we may retain signed consent forms and associated audit records after deleting other account information. We will restrict retained information to authorized purposes and will delete or de-identify it when retention is no longer reasonably necessary or legally permitted.

We may need to verify your identity and authority before responding. An authorized agent may submit a request where permitted by law, but we may require proof of authorization and identity. We will respond within the time required by applicable law.
If we deny a request, Colorado residents covered by the Colorado Privacy Act may appeal by replying to our decision with the subject line "Privacy Appeal." We will explain our decision and any available method for contacting the Colorado Attorney General.

The Services are intended for adults and are not directed to children under 13. We do not knowingly collect personal information directly from children under 13. If you believe a child has provided personal information to us, contact us so we can review and take appropriate action. Purchases must be made by a person who is at least 18 years old or the age of legal majority in that person’s jurisdiction.
